To work effectively within Adult Rehabilitative Mental Health Services and Outreach services that help clients to live more successfully and independently in the community.  Have knowledge of, and be able to teach skills related to coping, relapse prevention, and basic household and living skills. Qualifications:

Provide Adult Mental Health Rehabilitative Services and Community Support Program Services to adults with mental illness. Teach the client skills essential for managing his or her illness, and instruct, assist, and support a recipient in areas including but not limited to: interpersonal communications skills, community resource and integration skills, relapse prevention, budgeting and shopping skills, and household mgmt. skills. Most services are provided in the client’s home or community.

Qualifications:

MINIMUM QUALIFICATIONS: Minimum qualifications for the Rehab worker are AA degree in Behavioral Science, or 4000 hours supervised paid work experience in delivery of mental health services, or 3 years as a primary care giver to an adult with mental illness, or 3 years personal experience with mental illness. Must have a valid driver’s license and a reliable vehicle. Must have experience working with adults with mental illness.

DESIRED QUALIFICATIONS: Experience in providing services to adults with serious and persistent mental illness. Good writing skills, ability to assess a variety of situations and complete required paperwork; knowledge about chemical health issues and community resources.

Employment Specialist — Duluth/Cloquet

Homeless Service Coordinator Memorial Park Apartments

Full time Service Coordinator for Memorial Park to work as part of a multi-disciplinary team, to provide case management; crisis intervention; program intakes; assistance in obtaining benefits, and other supportive services for individuals who reside in the building. Adult Case Manager – Duluth, MN

SUMMARY: Case Managers maintain responsibility for a designated caseload – up to 30, coordinating and providing services for clients who have been diagnosed with a serious and persistent mental illness. We operate according to the guidelines of Targeted Case Management established by the Minnesota Department of Human Services.

 

DUTIES: Accept assigned referrals and maintain caseload. Complete required assessments including the LOCUS, Functional Assessment, Individual Community Support Plan, and all other required documentation. Arrange meetings with client and other providers and coordinate needed client services in areas of mental health, housing, employment, transportation and other basic needs. Work cooperatively with other service providers and understand St. Louis County program eligibility requirements and all referral aspects associated with foster home, board and lodge, and other placement options.

Qualifications:

MINIMUM QUALIFICATIONS: B.A. degree in human services plus 2000 hours of supervised clinical experience (one year), reliable transportation, proof of vehicle insurance, and valid class D drivers license. Knowledge of resources in St. Louis County.

DESIRED QUALIFICATIONS: Minimum of 1 year related experience. Experience working with individuals with mental illness and families. Experience in the placement of individuals in community options including foster care and board and lodge is helpful. Experience with employment options for the person with a serious and persistent mental illness.
