Case Manager Team Full-Time

SUMMARY: Full time Targeted Case Management position at HDC’s Homeless Program. Case Managers maintain responsibility for a designated caseload, coordinating and providing services for clients who have been diagnosed with a serious and persistent mental illness. We operate according to the guidelines of Targeted Case Management established by the Minnesota Department of Human Services. This position may also provide Adult Rehabilitative Mental Health Services to help clients live successfully and independently in the community.

DUTIES:  Accept assigned referrals, complete functional assessment and treatment plan with client, arrange and coordinate needed client services in areas of mental health, housing, employment, transportation and other basic needs, transport clients as needed, work cooperatively with other service providers and coworkers, complete progress notes and other duties as required. Be able to maintain appropriate boundaries in a variety of settings and situations.  Maintain a good working relationship with colleagues.

Qualifications:

MINIMUM QUALIFICATIONS: Bachelor's degree in Human Services or related field and 2000 supervised hours of mental health experience, reliable transportation, proof of vehicle insurance, and valid driver’s license.

DESIRED QUALIFICATIONS: Experience working with individuals who are homeless. Knowledge of community resources and area providers. Good time management skills, ability to be self directed and disciplined. The ability to apply creative solutions to difficult problem areas.

Social Service Aide – Douglas County

Salary / Wage: $12.00/hour

Summary:  Employee will act as an aide to social workers within the Department of Health and Human Services of Douglas County. The Social Service Aide will work as a part of a team providing services to adults and children involved with child protective services. The Aide will be housed in the County building in Superior, but will be employed by and report to the supervisor at HDC. It is necessary to have the Aide on site at the county to better help the social workers with emergencies that arise.

 

DUTIES:  Accept assigned referrals; assist with and organize supervised visitations, complete necessary paperwork, transport children to and from foster homes and/or visits, help in emergency placement situations. Work cooperatively with other service providers and coworkers.  Complete required documentation. May include working some evening and weekend hours as emergencies arise. The majority of the hours worked will be during the regular work week.

Qualifications:

MINIMUM QUALIFICATIONS: High school diploma or equivalent, with 1 year of experience. Requires driver’s license and ability to transport clients in own vehicle. Experience working with children is necessary. Experience with families in crisis and knowledge of community resources preferred.

Full-time Therapist – Carlton County

Salary / Wage: $43,953.30-54,739.94 (annual)

Description:

Provides psychotherapy to individuals, couples, families, and other groups including such activities as client diagnostic assessment, needs assessments, treatment planning, progress reporting and case review. Conducts initial interview and obtains client histories, obtains treatment records through client release of information, and gains consent to consult with other individuals regarding client treatment.  Defines treatment issues.  Provides assessment, consultation, and crisis intervention in conjunction with designated agencies such as social services, schools, medical facilities, courts, universities, law enforcement agencies, and individuals in the community.

Qualifications:

MINIMUM QUALIFICATIONS: Therapist based in the HDC Cloquet office. Requires licensure as a mental health professional (LICSW, LGSW, LP, or equivalent licensure), LICSW preferred. Diagnostic assessment experience and therapy skills are a must. Excellent communication and documentation skills and practices essential.

DESIRED QUALIFICATIONS: Generalist preferred. Therapy experiences with non-dominant culture and knowledge of Carlton County agencies and resources a plus. Ability to work on own, proactive attitude very important.

DESIRED QUALIFICATIONS: Master’s degree in human services field plus 3000 hours of verifiable post-master’s supervised clinical experience with adults diagnosed with serious and persistent mental illness, or 1500 hours of post-master’s experience working in a Community Support Program.

Psychiatric Mental Health Nurse Practitioner or Clinical Nurse Specialist

DESIRED QUALIFICATIONS: DESIRED QUALIFICATIONS: DNP degree a plus. Ability to treat children and adults is desired, but not required. Previous work experience in a community mental health clinic or other psychiatric clinic setting and in primary care settings as a NP/CNS. Also, knowledge of addictions and experience working with adults as well as other age populations; knowledge of community psychiatric systems and resources. Minimum of five (5) year of experience as a psychiatric nurse.
