Children’s Mental Health Case Manager/Children’s Therapeutic Services and Supports Practitioner (St.Louis County/Carlton County)

Salary / Wage: $14.49/Hour

Description:

Assist families in accessing necessary services to meet mental health issues of children ages 0-18.  Provide individual, group, and family skills training based on the treatment plan.  Goals of the program are to improve the functioning of children in home, in school and in the community.  Requires flexible scheduling and some evening work in order to meet with families in their homes.  Great opportunity to work with a multidisciplinary team including therapists, psychologists, and child psychiatry.

Qualifications:

MINIMUM QUALIFICATIONS: Bachelor’s degree in human services and at least one year’s experience providing mental health services to children (2000 hrs), or Master’s degree in human services completed or in progress. Ability to conduct and teach therapeutic skills in an individual, group, and family setting. Must be able to follow clinical direction of mental health professional and tailor skills services to unique needs of the child. Must be able to effectively teach social skills, independent living skills, emotional regulation, and basic parenting skills. Ability to work with schools, children, and families to address mental health issues. Good verbal and written communication skills.

DESIRED QUALIFICATIONS: Experience working collaboratively with a mental health team. Experience teaching and/or providing mental health services to chronically stressed families.

Homeless Case Manager Team – St. Louis County

Salary / Wage: $14.49/ hour

Description:

Full time position at HDC’s Homeless Project located in Duluth, Minnesota: be part of a multi-disciplinary team providing intensive case management, crisis intervention, mental health, housing, street outreach, housing, and supportive services for homeless and at risk individuals with mental illness and/or a dual diagnosis of MI and CD.  Must be able to effectively manage crisis situations, work independently, communicate with individuals and groups, provide advocacy for clients, maintain a good working relationship with colleagues, police and corrections, and various community organizations providing services to homeless persons.

 

Qualifications:

MINIMUM QUALIFICATIONS: Bachelor’s degree in Human Services or related field and 2000 hours of supervised mental health experience or HS diploma and 6000 hours of relevant experience. Requires excellent documentation skills, advocacy and community involvement in housing and homeless issues, a valid driver’s license and the ability to transport clients in own car, and grant reporting.

DESIRED QUALIFICATIONS: Cultural competence serving diverse population. Experience working with homeless persons or those at risk of becoming homeless with mental illness and/or dual diagnosis of MI and CD. Experience with housing programs (Bridges, Shelter Plus Care, Public Housing) and funding sources (HUD, PATH, ARMHS) preferred. Must understand and embrace the principles based on the Housing First and Harm Reduction Models. Non-judgmental, team player, outgoing, and compassionate.

Coordinated Services Team Coordinator

Summary: Provide family-based services as outlined in Wisconsin Statutes 46.46 to children and families by leading the Coordinated Services Team (CST). Act as lead in developing policies and procedures, building an effective collaborative team, and developing plans of care for each client that outline responsibilities of each team member, a timeline, and outcome expectations.

Duties: Ensure completion of quarterly reports, including the collection and recording of data, to evaluate the effectiveness of care. Ensure appropriate disposition of referrals, determine eligibility, and monitor services. Provide community-based case management service; outreach, aftercare, follow up; and professional consultation and communication education as requested. Perform duties and in eWiSACWIS in matters of investigations, client case management, social work plans, client progress notes, and other required records.

Qualifications:

MINIMUM QUALIFICATIONS: Minimum Qualification: A Bachelor's degree is required with preference for social work, psychology or other closely-related field. Vehicle required; Mileage reimbursement available.

DESIRED QUALIFICATIONS: Desired Qualifications: 5 years experience in the Human Service Field. Strong organizational skills, knowledge of computers, experience working in and on professional committees.

ARMHS/CTSS Grand Marais Office

SUMMARY: This position provides  Adult Rehabilitative Mental Health Services to help clients live successfully and independently in the community. This position may also provide CTSS – Children’s Support Services to children age 0-18.

Social Service Aide (Superior Office)

Summary:  Employee will act as an aide to social workers within the Department of Health and Human Services of Douglas County. The Social Service Aide will work as a part of a team providing services to adults and children involved with child protective services. The Aide will be housed in the County building in Superior, but will be employed by and report to the supervisor at HDC. It is necessary to have the Aide on site at the county to better help the social workers with emergencies that arise.

 DUTIES:  Accept assigned referrals; assist with and organize supervised visitations, complete necessary paperwork, transport children to and from foster homes and/or visits, help in emergency placement situations. Work cooperatively with other service providers and coworkers.  Complete required documentation. May include working some evening and weekend hours as emergencies arise. The majority of the hours worked will be during the regular work week.

Qualifications:

High school diploma or equivalent, with 1 year of experience. Requires driver’s license and ability to transport clients in own vehicle. Experience working with children is necessary. Experience with families in crisis and knowledge of community resources preferred.
