| Description | SUMMARY: Case Manager for Assertive Community Treatment Team (ACT) to work as part of multidisciplinary team providing intensive services to adults with mental illness. Small staff/client ratio providing services in clients’ homes and the community to prevent hospitalization and maximize community integration. DUTIES: As part of a close-knit team, accept assigned referrals, complete functional assessment and treatment plan with client. Arrange, coordinate, and provide needed client services in areas of mental health, housing, employment, and other basic needs. Provide own transportation to appointments, transport clients as needed. Work cooperatively with other service providers and coworkers. Complete progress notes and other documentation required. May include working early evening hours on some occasions, and sharing weekend coverage. |
| Qualifications | MINIMUM QUALIFICATIONS: BA degree in human service degree and 2000 hours experience providing mental health services. Requires driver’s license and ability to transport clients in own vehicle. The ACT team services are available 7:30 am to 6 pm 5 days a week with periodic half-day coverage on weekends.
DESIRED QUALIFICATIONS: Experience doing group work preferred. Experience with serving clients with dual diagnosis (mental illness, chemical dependency) preferred. Cultural competence serving Native Americans and/or African Americans preferred. |

Offer all supportive services for outpatient therapy, psychiatry, chemical dependency and children’s programs.

| Description | Full time position at HDC’s Homeless Project located in Duluth, Minnesota: be part of a multi-disciplinary team providing intensive case management, crisis intervention, mental health, housing, street outreach, ARMHS, housing and supportive services for homeless and at risk individuals with mental illness and/or a dual diagnosis of MI and CD. Must be able to effectively manage crisis situations, work independently, communicate with individuals and groups, provide advocacy for clients, maintain a good working relationship with colleagues, police and corrections, and various community organizations providing services to homeless persons. |
| Qualifications | MINIMUM QUALIFICATIONS: Bachelor’s degree in Human Services or related field and 2000 hours of supervised mental health experience or 6000 total hours of documented mental health experience. Requires excellent documentation skills, advocacy and community involvement in housing and homeless issues, a valid driver’s license and the ability to transport clients in own car, and grant reporting. Position may require working occasional Saturdays.
DESIRED QUALIFICATIONS: Cultural competence serving diverse population. Experience working with homeless persons or those at risk of becoming homeless with mental illness and/or dual diagnosis of MI and CD. Experience with housing programs (Bridges, Shelter Plus Care, Public Housing) and funding sources (HUD, PATH, ARMHS) preferred. Must understand and embrace the principles based on the Housing First and Harm Reduction Models. Non-judgmental, team player, outgoing, and compassionate.

| Description | DUTIES: Maintain credentialing compliance with all regulatory, accrediting and membership based organizations. Create and carry out various credentialing processes in relation to Psychiatrists, Advanced Practice Registered Nurses, and Psychotherapists.
*Process initial and reappointment applications, checking for completeness and accuracy. *Collect and process verification and accreditation information. This information could include pertinent education, training, experience and licensure content. *Enter information into the Intellicred credentialing software and process necessary applications for initial credentialing and scan any documents they provide for you (example: license, degree, resume, etc.). *Meet with clinicians. *Follow up with insurance companies *Send approval letters via email *Problem solve any issues with credentialing or denials. *Work closely with billing supervisor on denials in payment etc.

| Qualifications | MINIMUM QUALIFICATIONS: High School Diploma and three years experience in credentialing or a combination of additional education and experience.
Desired Qualifications: Experience in healthcare billing, insurance and credentialing desired.
