MN ACT Program
Assertive Community Treatment (ACT) teams provide the most intensive community services available. Services are provided to clients with a SMI or SPMI diagnosis who are referred by the St. Louis County or Carlton County Social Services mental health units. Staff to client ratio is 1/10, services are available 7 days a week, and include but are not limited to, medication management, psychiatry, case management, substance abuse treatment, crisis intervention, employment support, independent living skills training, and housing support. Services are based on individual needs of the client and provided primarily in the client’s home.
